Follow the instructions below to create an online account for your new insurance plan.

Aetna Instructions

To register a new account with Aetna, follow these steps:

  1. https://secure.bswift.com/login/first_time_user.aspx

  2. Click "Employee / HR Admin."

  3. Complete required fields.

Anthem Instructions

To register a new account with Anthem, follow these steps:

  1. https://employer.anthem.com/eea/public/registration

  2. Enter in your Group Number.

  3. Enter Tax ID and Zip Code.

  4. Type in User Information

  5. Complete User Access

  6. Complete Billing Access

Humana Instructions

To register a new account with Humana, follow these steps:

  1. https://account.humana.com/registration

  2. Select "Start Activation Now."

  3. Select "All other members."

  4. Type in Member ID, Date of Birth and Zip Code.

  5. Click "Continue" and follow the prompts.

To register a new account with Medical Mutual, follow these steps:

  1. https://member.medmutual.com/registration

  2. Type in Member ID, First Name, Last Name, and Date of Birth.

  3. Click "Continue" and follow the prompts.

United Healthcare Instructions

To search for providers within United Healthcare's Choice Plus network, follow these steps:

  1. https://www.healthsafe-id.com/rt/register/myuhc/en

  2. Type in First Name, Last Name, and Date of Birth.

  3. Type in Member ID and Policy Number.

  4. Click "Continue" and follow the prompts.

Dental Care Plus Instructions

To register a new account with Dental Care Plus, follow these steps:

  1. https://connectsso.dentaquest.com/authsso/selreg/RegisterDCPGView

  2. Type in Member ID, Date of Birth and email address.

  3. Follow the registration steps sent to your email.

To register a new account with Delta Dental, follow these steps:

  1. https://identity.deltadental.com/subscriber_registration_controller.jsp

  2. Type in First Name, Last Name, Member ID and Date of Birth and Zip Code.

  3. Click "Register."

To register a new account with Ameritas, follow these steps:

  1. https://www.ameritas.com/service/register.asp

  2. Complete "Contact Information" requirements.

  3. Complete "Choose your User ID and Password" requirements.

  4. Complete "Account Verification" requirements

Principal Dental instructions

To register a new account with Principal, follow these steps:

  1. https://secure05.principal.com/enterprise/registration?at=m

  2. Type in First Name, Last Name, Date of Birth, Phone Number and Zip Code.

  3. Click "Create Account."

Superior Dental  Instructions

To register a new account with Delta Dental, follow these steps:

  1. https://sdc.superiordental.com/Account.aspx/Register

  2. Create a unique username and then type in email and password.

  3. Select "Member" & click "Register."

  4. Type in Member ID, Date of Birth, and Group Number.

VSP Vision Instructions

To register a new account with VSP, follow these steps:

  1. https://www.vsp.com/create-account

  2. Type in last 4 of SSN

  3. Complete "Primary Member Information" requirements.

  4. Choose a username and password.

  5. Click "Create an Account."

To register a new account with EyeMed, follow these steps:

  1. https://member.eyemedvisioncare.com/member/en/register

  2. Type in First Name, Last Name, Date of Birth and Member ID/last 4 of SSN

  3. Type in email address

  4. Click "Create an Account."
